At SJHH, we transplant 120-165 patients annually including living and deceased donor transplants.The Renal Transplant Clinic focuses on preparing patients with chronic kidney disease for their upcoming kidney transplant surgery. We also work closely with living kidney donors to prepare them for their kidney donation surgery. Once surgery has taken place, transplant patients return to the clinic for follow-up care, management, support, and education.The Renal Transplant Clinic is a dynamic multidisciplinary team consisting of Registered Nurses, Nurse Practitioners, Physicians, Psychologists, Pharmacists, Social Workers, Registered Diabetes Educators, Dietitians, and Administrative Professionals.POSITION SUMMARY:We are seeking a highly motivated and experienced Registered Nurse to join our Renal Transplant Clinic as a Primary Care Nurse. The Registered Nurse (RN)



is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission,vision and values, policies and procedures; and applicable specialty standards.The Primary Care Nurse is an integral part of the team responsible for the care management of patients donating or receiving kidney transplants. The role of the Primary Care Nurse includes but is not limited to:Coordinating the care of patients from the time of referral throughout the transplant journey (encompassing initial assessment, listing, follow-up, donation/transplantation, and post-transplant/donation care)Providing patient education and supporting the patients understanding of the transplant journeyActing as the key contact person for patients/families, referring physicians, and other healthcare providers both internally and externallyCollaborating and participating in initiatives locally and with partner organizations such as Trillium Gift of Life Network, Canadian Blood Services, and the Ontario Renal NetworkQUALIFICATIONS:Current registration with the College of Nurses of Ontario requiredMinimum of two (2) years of nephrology, dialysis, or transplant nursing experience requiredDemonstrated recent leadership experience in renal or related nursing required, as evidenced through relevant roles, education, and/or professional activities,



including but not limited to:Experience in Charge Nurse and/or Team Lead rolesCompletion of additional nephrology or transplant-related education (e.G. courses/certificate, cNeph)Active participation in, or leadership of, committeesDelivery of nephrology or transplant-related educational presentationsCurrent BCLS/HCP certification requiredMinimum of one (1) year of transplant experience preferredCanadian Nurses Association certification in Nephrology cNeph(C) preferredMembership with RNAO or another relevant professional association preferredCurrent CPI and/or GPA training preferredExcellent attendance record requiredA resume outlining experience, further education, and any special courses is required and must accompany the applicationDEMONSTRATED KNOWLEDGE, SKILLS AND ABILITIES:Advanced case management, organization, time management, and coordination skills with the ability to prioritize workload to meet urgent and rapidly changing timelinesSelf-directed with strong leadership, problem solving, decision making, critical thinking, team building and interpersonal skillsDemonstrated ability to effectively communicate and provide education to patients and familiesDemonstrated ability to practice collaboratively and communicate clearly with members of the interdisciplinary team to provide care for a range of patients/clientsDemonstrated proficiency with computers (e.G., Microsoft Office [Word, Outlook, Excel, etc.], internet browser searches, data entry and patient information systems)Demonstrated commitment to ongoing learning, career development and continuous quality improvementHow To ApplyCARE members of the STARS or PASS Program should connect with their Case Manager before applying for this position.#J-18808-Ljbffr
