Provide support and skills training for client independence
Assess and strategize solutions for client issues
Ensure clients meet their basic needs through networking
Maintain documentation and create statistical records
Uphold health and safety standards in all duties Requirements:
Relevant Diploma or equivalent combination of experience
Experience in case management, minimum one year
Occupational First Aid Level 1 Certification
Class 5 Driver’s Licence is required
