Collaborate with the interdisciplinary care team
Provide patient-centered symptom management




Coordinate care and create personalized care plans
Participate in clinical huddles and case conferences
Travel within the assigned region to provide care Requirements:
Registered with the College of Nurses of Ontario
Minimum two years of nursing experience
Experience in palliative care preferred
Current CPR and First Aid certification
