As a vital member of the leadership team at Extendicare, you will work closely with the Administrator to consult on therapy program needs and implementation.

Your role will involve assessing residents' needs, developing tailored therapy treatment plans, and actively participating in initiatives such as fall prevention and no restraint policies. Key Responsibilities:
Plan and implement occupational rehabilitation programs
Develop individualized therapy treatment plans
Recommend adaptive equipment for residents
Conduct assessments per MDS 2.0 requirements
Collaborate with the Interdisciplinary Team on care planning Requirements:
Degree in Occupational Therapy from an accredited school
Active registration with ACOT and CAOT
Preferred experience in long-term care
Proficiency in Microsoft Office applications
Ability to foster effective team relationships Lead quality care initiatives and empower residents at Extendicare, making a meaningful difference in their lives.
