Leverage your skills to coordinate supportive services and community integration full time. Working at PACE, a nonprofit dedicated to helping over 350 individuals live independently, you will facilitate effective service planning and enhance client well-being.
Your role will bridge gaps between clients, healthcare providers, and community services, ultimately improving access to essential resources. Make a difference with your expertise in community support. Key Responsibilities:
Empower clients through life skills coaching
Develop partnerships with community organizations
Coordinate client transitions across care settings
Share educational resources for informed client decisions
Maintain program quality and accurate client records Requirements:
Post-secondary education in a relevant area
3 to 5 years experience in community support
Proficiency in case management and system navigation
Awareness of AODA and health service standards
Essential conflict resolution and empathy skills Help PACE transform lives through dedicated community navigation and support services.
