You will plan, implement, and adjust care plans with clients and families, coordinating with physicians, facilities, and other programs.

The role involves mobile travel to Didsbury, Airdrie and surrounding rural areas, operating a laptop and mobile phone, and performing manual handling as part of daily duties.
