LOFT is also recognized as a Nonprofit Employer of Choice™ Award recipient.Job SummaryAs the Registered Practical Nurse, you will be part of an interdisciplinary team, expected to ensure the safety and security of the resident environment, practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services. The RPN will work with individuals from diverse backgrounds, contribute to program development, participate in clinical supervision, and provide support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The RPN is directly responsible for developing and articulating strategies for the management of recovery and psychosocial rehabilitation using information collected through assessments and collaboration with the PSWs. Additionally, you will liaise with community agencies, other practitioners, and key partners to coordinate care for clients.The Path Home – Pine Villais a short‑term transitional housing program for hospital patients who no longer require acute care but still require care and support in preparation for a long‑term living arrangement. Pine Villa specializes in supporting older adults (55+) with complex physical health challenges, mental health challenges, addiction or cognitive impairment. The Path Home – Pine Villa provides client‑centered and goal‑oriented support services that enable clients to continue their recovery in the community as they wait for permanent housing.The Path Home – Pine Villais a unique partnership between LOFT Community Services, SPRINT Senior Care and Sunnybrook Life Sciences Centre. This partnership reflects the transformation of health care in Ontario which brings the expertise and some of the resources of a hospital setting to a community‑based transitional housing program. Some of these resources include on‑site clinical practitioners including an Occupational Therapist, Physiotherapist, Nurse Practitioner and Advanced Practice Nurse.What You Will DoProvide direct nursing care with medications as defined by the College of Nursing Practice including:Administration of medications through oral, subcutaneous, topical,



natural body orifices, intracutaneous, tubing routesTranscribing medical orders as given by physicians in writing or by telephoneAdministration of routine medications including narcoticsAdministration of routine p.R.N. medications, effect of such and identify when the order needs to be adjustedMaintain security of medication system and storageOrder medications as required and maintain the MAR systemFollow all policy and procedures as outlined by the PharmacyIdentify changes in appearance/behaviours, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviourParticipate in service planning and development of personal goals for clients to transition to their permanent discharge locationProvide support with behaviours arising from cognitive issues, intervening in conflicts between residents, and contacting appropriate emergency services in crisis situationsMaintain all records of service and ensuring supplies and equipment are readily availableConduct mandatory assessments and provide timely and accurate record‑keeping and reports as requiredEnsure proper care is rendered in a safe and clean environmentLiaise with physicians, pharmacists and other health care providers regarding client careProvide advice, coaching and support to staff regarding medical/healthcare questions and concernsDevelop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principlesMaintain strict confidentiality and follow all other required policies and proceduresConsistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on LOFT Community ServicesLiaise, collaborate and coordinate with other professionals, service providers and partners within the ProgramParticipate in on‑call rotation regularly and as requiredFollow standard infection control practices and enhanced precautions as instructedEffectively participate as a member of the LOFT Team; attend staff/teammeetings, client care conferences, other meetings etc. as requiredServe as back‑up to Personal Support Workers, as requiredAttend and actively participate in all required trainingComplete other duties, from time to time,
