Take on the role of HIV System Navigator at Positive Living Niagara, focusing on client support and health service coordination. In this position, the System Navigator will provide vital case management and assistance to individuals living with HIV, helping them navigate complex healthcare systems.

Your efforts will be directed towards fostering robust relationships with community partners and enhancing the quality of life for clients through informed decision-making and support. A commitment to inclusive, trauma-informed care is essential. Key Responsibilities:
Coordinate client services across healthcare systems
Offer direct referrals for HIV treatment options




Conduct regular reviews of client goals and progress
Assist clients with understanding community resources
Promote harm reduction principles and health education Requirements:
Degree in a relevant field, such as social services
At least 2 years in direct client support
Valid G Ontario Driver’s License; access to a vehicle
Recent vulnerable sector check required
