Join The Health Centre as a Primary Care Connection Lead in Waterloo Region and enhance health service access for the community. This full time role focuses on client intake and coordination. You will be responsible for directing the intake processes and ensuring timely, effective triage for clients requiring health services. As a leader, you will work collaboratively with healthcare teams and partners to identify gaps in service and drive strategic improvements. Key Responsibilities:
Oversee and coordinate intake and referral workflows
Conduct assessments for client eligibility and service level
Maintain accurate health records in compliance with policies
Engage with external healthcare networks and community organizations
Report on key performance indicators for intake success Requirements:
Must hold a registration as a Nurse in Ontario
At least two years’ experience in relevant health settings
Experience in triage and decision-making processes
Familiarity with community health resources
Solid interpersonal skills and team collaboration ability Lead comprehensive client support and improve health outcomes as part of a dedicated team.
