Elevated Health is a beautiful integrated medical clinic located at 933 17th Ave. SW in Calgary. We currently have a Nurse Practitioner, three Naturopathic Doctors, and a Holistic Nutritionist. We are both integrated and integrative so that patients have access to every aspect of care they need. We are looking for another part time NP to support our patients with general functional medicine and BHRT.

Duties include:
Conduct comprehensive health assessments and provide primary care to patients from a functional medicine perspective
Support patients with bioidentical hormone replacement therapy (BHRT)
Collaborate with other healthcare professionals (including ND's) to develop and implement patient care plans
Utilize Jane and Ava EMR system for documentation and patient management

Skills:
Strong clinical skills in functional medicine
Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
Ability to provide health coaching and education to promote patient self-care and disease prevention
