Make a meaningful impact in your community as a Connection Coordinator with House of Friendship. This position promotes individual recovery through strengths-based care and community collaboration. In this full time role within the HART Hub program, you will work directly with participants facing homelessness and substance use issues in the Waterloo Region, focusing predominantly on Cambridge.
Your efforts will center on building supportive relationships, creating individualized care plans, and ensuring access to essential resources. Engage with community agencies and contribute to the recovery journey of each participant. Key Responsibilities:
Connect and engage with program participants effectively
Develop and implement individual support plans
Support participants with harm reduction education
Collaborate with interdisciplinary teams for optimal care
Conduct case conferences to align efforts across stakeholders Requirements:
Minimum College Diploma or related degree
At least 2 years of relevant case management experience
Robust knowledge of addiction and recovery principles
Excellent communication and facilitation skills
