Your role will require excellent communication and support skills. Key Responsibilities:
Establish rapport with clients facing complex challenges
Conduct thorough assessments and craft personalized support plans
Provide behavioral support using a trauma-informed approach
Assist individuals in navigating community services effectively
Collaborate with a range of community partners for continuity of care Requirements:
Degree or diploma in Social Work, Psychology, or equivalent
At least three years’ experience in a related field
Crisis intervention training such as ASIST or Mental Health First Aid
Familiarity with local mental health and housing support systems
