Maintain accurate documentation and update care plans




Coordinate with physicians and service providers on care strategies
Provide direct nursing care and medication management
Assist in resident transitions and discharge planning
Support infection prevention and quality improvement initiatives Requirements:
Valid LPN registration in British Columbia
Prior experience in long-term care preferred
Solid communication and critical thinking skills
CPR and First Aid certification
