This permanent position involves collaboration within the Community Services - ACT department. As an Outreach Worker, you will engage proactively with clients, offering a variety of services aligned with individual needs.

Your role emphasizes relationship building and utilizes a client-centered approach, addressing social determinants of health and promoting health equity. Key Responsibilities:
Deliver support and recovery services based on individualized service plans
Engage with recent and existing clients to facilitate skill-building




Advocate for clients using a non-judgmental and culturally appropriate manner
Collaborate within an interdisciplinary team to achieve service goals
Apply harm reduction and trauma-informed practices for client care Requirements:
Valid Driver’s License and access to reliable transportation
Minimum 2 years’ experience with community programs
Proven skills in crisis management and problem-solving
Understanding of Indigenous cultures and their impacts
