Carefirst is hiring a Registered Nurse (RN) or Registered Practical Nurse (RPN) for their dedicated Home Care Team in Scarborough. This role offers the chance to make a meaningful difference in program delivery while collaborating with a passionate team serving over 15,000 clients annually. Key Responsibilities:
Assess client needs and deliver case management support
Serve as a clinical consultant for complex health cases
Submit Initial and Interim Client Assessment Reports
Attend meetings for continuous quality program improvement
Offer advice to the Infection Prevention and Control Committee Requirements:
Nursing or Practical Nursing degree/diploma required
At least 3 years of community healthcare experience
Active member of the College of Nurses of Ontario
Current First Aid & CPR certification
