In this role, you will implement person-centred care models, conducting assessments and developing care plans tailored to clients' needs. Engage in diverse nursing practices such as chronic disease management, advanced wound care, and medication management.

Driving is necessary as you will travel to meet clients where they are.

Key Responsibilities:
Assess clients to formulate personalized care plans
Collaborate with healthcare specialists for primary care
Provide advanced wound care and manage conditions
Administer IV therapy and monitor client health
Handle phlebotomy and other critical nursing tasks

Requirements:
Must be a licensed Registered Nurse in NS
Experience in community health is a plus
Personal vehicle and valid driver’s license required
Must pass a Criminal Records Check
Capable of wearing PPE during care
