Registered Practical Nurse (RPN)

SCH Home Health is seeking a reliable and compassionate registered practical nurse to join our community home care team as an independent contractor providing ongoing home nursing assignments based on patient care needs.

The RPN will provide scheduled nursing visits to patients in their homes and work closely with our care coordination team. This is an ongoing contract opportunity with morning, daytime, and occasional evening assignments based on patient care needs. Flexible availability across multiple days per week is preferred to support patient scheduling.

Responsibilities:

- Provide nursing assessments and care during scheduled home dialysis visits
- Support patients with peritoneal dialysis (PD) care, treatment monitoring, and reinforcement of teaching
- Monitor vital signs, weight, fluid status, drainage/effluent, and overall patient condition
- Identify and report changes in condition, treatment concerns, or complications promptly
- Provide nursing interventions according to the care plan, physician orders, and scope of practice
- Complete accurate and timely clinical documentation
- Communicate with patients, families, healthcare providers, and the SCH Home Health team
- Follow infection prevention, dialysis safety, and organizational policies

Requirements:

- Current registration in good standing with the College of Nurses of Ontario (CNO)




- Valid Vulnerable Sector Check or willingness to obtain one
- Valid TB test
- Strong clinical assessment and documentation skills
- Ability to work independently in a home/community setting
- Professional, reliable, and punctual
- Must have valid driver’s licence
- Must have Reliable vehicle
- Solid communication and critical-thinking skills
- Availability for morning, daytime, and occasional evening assignment is preferred.

Preferred Experience:

- Home care or community nursing
- Hospital or long-term care experience
- Experience caring for medically complex patients
- Renal or dialysis experience is an asset but not required

Additional Requirements:

- Comfortable travelling between patient homes
- Ability to travel to patient homes across Durham Region

Job Type: Contract Schedule: Ongoing assignments, typically mornings, daytime, and occasional evenings based on patient care needs.

Compensation

Starting $28
