Operating under the Home and Community Care Act, Hillcrest offers a short period of reactivation to clients who are ALC or at risk of ALC, supporting their return home.DutiesInnovative application of social work processes that assist patients with psychosocial challenges and optimises client progress in rehabilitation through the continuumProviding comprehensive psychosocial assessments and interventionProviding educational and supportive counselling services; providing servicebasedon individual, family and group practice modelsDischarge planning, population health advocacy, community services linkageCollaborating with the inter‑qualified team and community‑based service providersAdvocating, mediating and resolving conflictParticipating in professional,
