Join our team as an MDS RN Coordinator to facilitate effective MDS assessments and care planning. This supervisory position requires acute attention to detail and strong leadership skills.

As an MDS RN Coordinator, you'll take a leading role in managing the resident assessment process, ensuring accurate and timely completion of all MDS evaluations. You'll provide training to current staff and analyze performance metrics to improve clinical outcomes. Your ability to coordinate interdisciplinary efforts will be essential to ensure comprehensive resident care plans are established and maintained throughout their stay.

Key Responsibilities:
• Ensure accurate completion of MDS assessments
• Communicate care levels across disciplines




• Schedule resident care conferences and reviews
• Maintain accurate assessment scheduling and records
• Generate forms for level acuity and changes

Requirements:
• Bachelor of Science in Nursing preferred
• Must hold current nursing license
• 3 years of supervisory experience preferred
• Familiarity with MDS and PDPM tools
• Experience in long-term care facilities
