Key Responsibilities:
• Collaborate with teams for resident transitions back to LTCHs
• Provide thorough assessments and follow-up care




• Deliver education to caregivers and LTC staff
• Promote patient-centered nursing through expert practice
• Implement programs to reduce acute admissions from LTCHs

Requirements:
• Baccalaureate Degree in Nursing
• RN in positive standing with College of Nurses of Ontario
• Minimum 3 years in acute medicine and geriatrics
• Current BCLS/CPR certification
• Geriatric Certificate Program required
