Support and care are provided in a team-based approach, including various interdisciplinary team members such as personal support workers, nursing, physicians, spiritual and psychosocial support. We embrace a person-centered and holistic model of care and the importance of providing individuals dignity at the end of their life.What You’ll Bring to Our TeamGraduate of an accredited school of nursing and registered in good standing with the College of Nurses of Ontario (required)BCLS certificate (required or willing to obtain upon hire)Experience working with individuals and their families in the end of life or with life-limiting illnessStrong clinical assessment skills and ability to work with evidence-based practices related to pain and symptom management and end-of-life care (required)Previous palliative care experience either in hospital-based palliative care,
