Job Purpose Under the direction of the Director of Settlement and FamilySupport Services and Unit Team Leads, the Complex Case and Outreach Counsellor isresponsible for supporting refugees facing unique or multiple medical andnon-medical barriers that require specialized support and multi-stakeholdercoordination. These cases go beyond routine settlement needs and involvecomplex family, legal, housing, medical, or trauma-related challenges thatimpact a client’s ability to integrate successfully. This role involves casemanagement, advocacy, and collaboration to connect clients with supportservices. This position assesses cases, develops service plans, and works withinternal teams, agencies, and community partners to ensure timely interventionsand lasting solutions. In addition to case management, the position designs andfacilitates specialized group workshops and delivers community outreach tosupport client independence and self-sufficiency. The position is frequentlyexpected to provide service in the client’s first language or a common languageother than English.

Duties and Responsibilities Complex Case Management

Supportsthe delivery of the Resettlement Assistance Program (RAP) for refugeesidentified as complex cases who are transitioning from RAP to Case Management.

Assessesand identifies complex cases referred internally, including those involvingmulti-system coordination, serious trauma, family crisis, housing barriers, ormulti-provider medical needs, determining appropriate referrals, risks, andinterventions.

Developsand implements individualized service plans in collaboration with clients andrelevant stakeholders, addressing client-specific medical or non-medicalchallenges.

Providescrisis intervention and conflict resolution as needed.

Coordinateswith healthcare providers, mental health services, and disability supports toensure a holistic approach to client care.

Accompaniesor arranges accompaniment and interpretation for medical appointments whererequired.

Providesguidance and support to clients facing challenging family circumstances, suchas unaccompanied minors, separated families,



domestic violence survivors, ormarital breakdowns.

Assistsclients in securing permanent accommodation, particularly for those needingaccessible housing, those receiving multiple landlord refusals, or largefamilies struggling to find suitable housing.

Advocatesfor clients within government programs, legal systems, housing services, andhealthcare systems to ensure equitable access to resources.

Maintainsdetailed and confidential case records, ensuring compliance with organizationaland regulatory standards, to track client progress, service referrals, andresolution strategies.

Evaluatesservice outcomes and provides recommendations for continuous improvement.

Keepsmanager/supervisor informed by regular reporting as required and advising themon emergent issues.

Workshop Facilitation

Designsand develops specialized group workshops for clients with complex needs.

Adaptsworkshop content and delivery methods to the literacy levels, languages,cultural backgrounds, and trauma experiences of participants.

Coordinatesworkshop logistics, including scheduling, venue, interpretation, childcare,transportation, and materials.

Deliversworkshops and arranges interpretation where required.

Collectsparticipant feedback and attendance data, and revises curriculum and deliverybased on outcomes and emerging client needs.

Sharesworkshop curricula and facilitation resources with Settlement Counsellors andCase Management Support Counsellors to strengthen standard programming.

Outreach and Community Engagement

Conductstargeted outreach and follow-up through home visits, community-based sessions,and repeated contact with clients on an active complex case caseload who areisolated, disengaged, or facing barriers that standard settlement outreach hasnot resolved.

Buildsand maintains relationships with external partners, including healthcareproviders, legal aid services, housing agencies, school divisions, familysupport organizations, and trauma support providers.

Promotesrespect for cultural diversity by informing and educating the community aboutimmigrant and cross-cultural issues.

Programming and Documentation





Providesprogram information and data by maintaining client records, preparing requiredclient files, and participating in staff meetings.

Participatesin training and capacity-building efforts to improve the organization’s abilityto address medical and non-medical complex cases.

Ensuresservice quality and collaborative work relationships by providing guidance tonew staff and acting as a resource person in an identified area of expertise.

SupportsSettlement Counsellors and Case Management Support Counsellors, whereapplicable.

Other duties as assigned.

Education and Experience

Bachelor’sdegree in social work, education psychology, social sciences, public health, ora related field and one year of work experience providing counselling services,OR

Post-secondaryeducation in humanities, community services or adult education and two years’work experience providing counselling services, OR

Anequivalent combination of direct-related training and experience.

Experiencein case management, crisis intervention, social services, or refugeeresettlement, preferably handling complex and high-needs cases.

Strongknowledge of trauma-informed care, family reunification processes, andlegal/social support systems.

Strongknowledge of community resources, healthcare services, and housing supports.

Abilityto assess risks, develop intervention plans, and coordinate multidisciplinarysupport for clients.

Demonstratedability to design and facilitate group workshops for adult learners fromdiverse cultural and linguistic backgrounds.

S trongproblem-solving, mediation, and negotiation skills.

Abilityto work collaboratively with government agencies, NGOs, legal serviceproviders, and healthcare professionals.

Strongability to work independently and collaboratively in high-pressureenvironments.

Proficiencyin documentation and case reporting.

Assets:

Experienceworking with diverse populations, including newcomers and refugees.

Trainingin life skills counselling, mental health first aid, conflict resolution, orcrisis intervention.

Proficiencyin a second language.

Required Licenses and/or Certifications

CriminalOccurrence Security Check – Must complete a Criminal Record and VulnerableSector Check in valuable standing and within three months prior to first day ofemployment.

Proof ofEnglish proficiency by obtaining a completed Canadian education, Minimum CLBlevel 8, or IELTS 6.5.
