Be a vital part of patient care as a Personal Support Worker at UHN's Queen's Estate. Provide essential support in daily living activities and ensure patient safety
This casual role at the Transitional Care Program requires you to assist patients with daily living tasks, such as bathing and feeding, while collaborating with RNs and RPNs. You will be responsible for maintaining patient records, demonstrating care plans, and reporting any safety concerns to ensure a secure environment.
Key Responsibilities: • Assist patients with daily living activities like feeding and transfers • Perform delegated tasks under RN/RPN supervision • Communicate basic instructions according to care plans • Report and document unsafe conditions or changes in patients • Maintain accurate patient records and progress notes
Requirements: • Current registration with HSCPOA • Basic Cardiac Life Support (BCLS) Certificate • Training in behavioral management is an asset • Experience in acute care or rehab considered beneficial • Robust computer skills and good attendance
