Position Summary As a

Registered Practical Nurse,

you’ll provide quality and compassionate care to individuals in their homes. Our team of Nurses are dedicated to making a difference where it matters most: at home. You'll use specialized skills to enhance the daily lives of clients, providing the specialized care that only nurses can offer. In this role, you aren't just a clinician-you are a trusted partner to families and a key collaborator in the healthcare journey to ensure safe, effective and meaningful care.

What You’ll Do Clinical Responsibilities:

Provide quality nursing care, within the scope of practice and in accordance with the CNO

Maintain knowledge and compliance to the Qualified Standards

Follow nursing processes and practices in an effective and efficient manner

Ensure the maintenance of high-quality nursing skills, applying current knowledge to nursing care

Comply with all SJHC nursing practices, processes and work instructions

Review and followthe plan of nursing care, including discharge plans

Engage in consultation / liaison with clients, their families/caregivers, physical, multi-discipline team member, and other external stakeholders as required (eg. HCCSS, ICC Care Coordinators)

Evaluate and monitorcare plans on an on-going basis

Assist other clinical programs if requested or required

Participate in regular and on-going professional development / learning opportunities, with the goal of obtaining more advanced or specialized nursing skills, as may be necessary for organizational advancement

Complete self-appraisals in keeping with the College of Nurses of Ontario Standards of Practice and SJHC internal practices

Act as an advocate for patients, families and their caregivers through reviewing and interpreting information, supporting decision making and facilitating communications

Conduct virtual care visits as necessary to program and/or scope of practice

Operational:

Complete program, clinical or organizational documentation in a timely and accurate manner

Participate in organizational learning opportunities as appropriate (e.g, Surge Learning)





Engage in organizational working groups as appropriate (e.g, JH≻, Quality Committee)

Effectively utilizeorganizational tools, software, devices in the application of daily work, willingness and ability to learn new technical skills as operational or patient needs change

Willingness and ability to provide care in a variety of geographical locations (eg. Central Hamilton), patient care environments (eg. apartment building), or institutional care settings (eg. Long Term Care)

Willingness and ability to act as coach/mentor/Preceptor to new nurses through knowledge sharing and peer-to-peer support

What You Bring Education & Credentials:

Diploma in Practical Nursing from an accredited college

Registered with the College of Nurses of Ontario (CNO)

Current BCLS/HCP certification

Experience:

1–3 years of clinical experience (required)

Experience with IV medication administration (required)

Home and community care experience (preferred)

Familiarity with mobile work tools and EMR systems

Are you available to work Mondays and Tuesdays + 1 additional day Monday-Friday, as well as every 1 in 3 weekends required for a PART-TIME position?*

We are a unionized environment and have Collective Agreements with ONA for RN's and LIUNA for RPN's. Within those Collective Agreements is the requirement to work a MINIMUM of 3 shifts per week Monday - Friday for PART-TIME positions AND 1 in 3 weekends for FULL-TIME, PART-TIME and CASUAL.*

What shifts are you available to work?*

It is a requirement to be available for the entire first week (5 days) for new hire orientation. Will you be able to commit to our new hire orientation for the first week (5 days)?*

If selected for an interview, you will be required to provide a MINIMUM of 2 weeks availability to the hiring manager during the interview for scheduling as part of our new hire onboarding. Are you able to provide 2 weeks of availability during your interview?*

Wages- SJHC is a unionized work environment, and we have a Collective Agreements in place for both RN's and RPN's. RN's are unionized with the ONA group. RN (ONA) wage band has 11 levels. $31.39-$46.47 (July 1st, 2025) RPN's are unionized with LIUNA Local 3000. RPN (LIUNA) wage band have 5 levels $27.52-$29.84 Each level, for both ONA and LIUNA, is equivalent to 1500 hours worked with a previous employer. Full Time Benefits Package: 6- Paid Sick Days 15-Vacation days Extended Health Care Package starts at 3 months PT Benefits Package: 6% in lieu of vacation for PT/ Casual Positions 6% in lieu of Benefits PT/ Casual Positions Driving time is paid as part of your day. SJHC currently pays .52 cent per km you drive in addition to hourly wage. Do you accept these wage grids?*

Would you be interested in taking additional shift visits? Shift visits will be a minimum of 3 hour visits with one client in their home and may include HNHB boundaries including Hamilton, Burlington, Brantford, Niagara regions and Norfolk County.*
