PRIMARY CARE NETWORKS - PROGRAM COORDINATORPermanent, full-time roleHybridSalary range: $55,000 - $59,000OrganizationThe Vancouver Division of Family Practice (VDoFP) is a not-for-profit organization with a mission to improve the primary care system in the City of Vancouver for the benefit of our family doctors and their patients. We are funded by the Government of British Columbia and Doctors of BC, and work in partnership with Vancouver Coastal Health, Providence Health Care, community health organizations and the BC Ministry of Health. Team VDoFP and VCH are collaboratively establishing a Ministry of Health-funded Primary Care Network (PCN) model to create an integrated system of care. In the Vancouver PCNs, family physicians and nurse practitioners (via Patient Medical Homes); registered nurses, allied health professionals, and other health care providers all work together to provide effective and comprehensive, patient-centered primary care services. The VDoFP PCN team works closely with the VCH PCN team, to build primary care networks in Vancouver that include the voices of all stakeholders. PositionReporting to the PCN Manager, the PCN Program Coordinator collaborates with the Community Network Managers (CNMs) to assist with implementing various projects, provide administrative and logistical support, create and maintain PCN dashboards and facilitate effective communication between stakeholders to support the successful development of the six PCNs in Vancouver. The work is very fluid and requires an adaptable, creative approach. Successful outcomes require the PCN Program Coordinator to work independently and in partnership with various teams across Vancouver. Additional duties may be assigned as the PCN program develops. Key Responsibilities & Duties Supporting PCN Team with outreach efforts, stakeholders’ engagement,



educational sessions and logistics:- Collaborate with the PCN team to develop educational materials and resources for orientation and training sessions.- Provide operational and administrative support to the PCN Manager and the broader PCN Team and work with other administrative staff as needed.- Organize and schedule meetings; work with stakeholders on agenda setting, prepare and distribute meeting material, identify, track, and follow up on action items. Document and Reporting:- Prepare regular reports summarizing the outcomes of events, key findings from qualitative and quantitative data analysis, and progress on implementing new tools and supports.- Collaborate with Engagement & Events Coordinator to maintain and update the PCN website, ensuring that all content is accurate, relevant, and visually appealing to engage the target audience effectively.- Maintain program tracking documents utilized by the VDoFP and VCH PCN teams (e.G. PCCP co-location, RN-clinic readiness), ensuring accuracy to support efficient resource allocation.- Monitor the PCN, VDoFP and VCH Master List of resources, hires, terminations, leaves, etc - Support the planning and implementation of PCN-specific and Vancouver-wide projects ensuring they meet the goals of the PCNs. - Prepare and coordinate surveys, documents, and presentations to support the work of the PCN team. PCN Communications (in collaboration with PCN Engagement and Events Coordinator):- Coordinate with the VDoFP and VCH PCN teams to gather information, news, updates, and transform them into compelling newsletters that effectively communicate the PCN’s activities, events, and initiatives.- Collaborate with the VDoFP communications team alongside Engagement & Events Coordinator to ensure relevant PCN stories are featured/linked to the Division’s membership website, connecting the work of the PCN and VDoFP membership.- Work closely with PCN Manager and PCN team, and the suite of patient medical home (PMH) teams (e.G.



Clinic Business Support team) to ensure effective communication and information sharing and streamlined project support between VDoFP teams. - Act as a key point of contact and liaison for the PCN and PMH teams to ensure there is a streamlined communication channel for all projects and services.Other duties, as required as the program continues to develop and evolve.Qualifications and skills: A successful PCN Program Coordinator brings the following to the role: - Bachelor's degree in a relevant field (e.G., Business Administration, Public Health, Health Sciences) or equivalent work experience. - Proven experience of at least 2 years in program administration, preferably within a PCN or healthcare-related setting. - Experience in data management, including data collection and reporting. - Proficiency in Microsoft Office suite (Word, Excel, PowerPoint) and familiarity with data management and project management software and tools. Experience with Power BI an asset. - Excellent communication skills, both written and verbal, with the ability to convey complex information clearly and concisely to various audiences. - Highly organized with exceptional attention to detail, ensuring accuracy and completeness in all program-related documentation and data. - Strong interpersonal skills, demonstrating empathy, cultural sensitivity, and the ability to work effectively with diverse populations and community members. - Flexibility and adaptability to handle multiple tasks and responsibilities in a fast-paced and dynamic environment. - Willingness to work a adaptable schedule with occasional (3-4 times monthly) early mornings or evenings, and occasional travel across the City of Vancouver.
