Fraser Health is seeking a dedicated Social Worker for the Community Care Team in Burnaby, B.C. This relief full time role focuses on providing psychosocial assessments and therapeutic services to clients.
As a member of the interdisciplinary team, your objective will be to promote self-determination while conducting thorough assessments and determining care plans. Your collaboration with community services will ensure that clients receive cohesive and effective care throughout the healthcare continuum.
Key Responsibilities:
Perform in-depth psychosocial assessments for effective treatment
Collaborate with clients and families for care plan development
Act as a resource for community service referrals
Maintain patient records per established standards
Engage in community planning to enhance service delivery
Requirements:
Current registration from the BC College of Social Workers
At least two years' experience in a clinical setting
Valid BC Driver's License might be necessary
Knowledge of First Nations health issues
Solid communication and collaboration skills
