Position Position Purpose Summary Schedule will vary according to requirements and responsibilities – evening and weekend work will be required. Shifts include a ½ hour paid meal break where applicable. Supervisor will set the hours of work and work schedule. Shifts can include the following: days, afternoons, evenings and weekends. Job Title Case Manager - Community Participant Supports (CPS) Responsibilities Plan and implement individual, goal-centered programs and offer support in workplace settings for adults with developmental disabilities participating within the CPS Lawson Ministries program and in the community. Provide programming that is supportive and nurturing to individuals that require support within the residential program where required. To promote greater independence and strive toward the highest quality of living possible. To promote an environment and programs that are supportive, nurturing and fostering independence with everyday living aimed at the person’s goals. Assist the person supported in meeting emotional and psychological needs. Involve in planning, implementation and evaluation of person-directed plans with persons supported, team members, other staff and professionals, and the evaluation of such plans. Ensure the person supported's physical needs (food, clothing, shelter) are well met and provide personal needs care as required. Promote inclusion within the person supported’s community.
Accountabilities Have working knowledge of the application and maintenance of person-directed plans along with SA policies and procedures for each person in the program. Organize transportation of clients to and from destinations as required. Train new staff and provide program orientation; work with programparticipants. Support efforts to meet persons supported spiritual needs by participating in programming activities where necessary. Understand psychological and social dynamics of working with adults with developmental diagnoses and behavioral needs. Work cooperatively with the day program team, support networks and residential counselors to provide coordinated approaches. Develop strong therapeutic relationships with persons supported and their families. Ensure proper and accurate recording of daily logs, shift reports and incident reports using appropriate systems. Accompany required professional appointments for persons supported and document as necessary. Trained in medication dispensing; responsible for administration, documentation and ordering of medications. Maintain cleanliness and upkeep of workspace and main office. Assist with client emergencies during shifts; perform other duties asassigned by Supervising staff, Director of Operation or Executive Director. Relationship Management Internal : Regular interaction with shift supervisor and team members; regular interaction with persons supported. External : Primary care support team; Community Partners. Leadership & Responsibilities This position reports directly to the Program Supervisor or Director. Uses or maintains organization assets in accordance with guidelines; no budgetary responsibilities.
Working Conditions Work in the community and office, or as assigned, in generally agreeable conditions. May be required to work after hours if business requires. Education, Experience, Qualifications Completion of a formal post-secondary/college program of two academic years: Developmental Social Worker Diploma or equivalent in Human Services field. Experience and training in FASD; experience accessing and networking with community resources. Current First Aid & CPR with Defibrillation and Non-Violent Crisis Intervention Certification; medication administration training and experience. Non-Violent Physical Crisis Intervention Certification required. Criminal Records Check (CRC) with vulnerable sector screening (through CPIC or local police); satisfactory backgroundcheck required. Valid Ontario Class G or G2 driver’s license (vehicle access may be required). WHMIS certification (2015) and any additional Salvation Army required trainings. Experience with adults with developmental disabilities and vulnerable populations (minimum 2 years); alternative combinations of experience and education may be considered. Skills & Capabilities Lifts and transfers are an asset; ability to lift up to 20 lbs. Strong oral and written communication; self-motivated, disciplined, positive and goal-oriented. Ability to work independently and as part of a team to provide a supportive and nurturing environment. Positive communication skills across verbal, written and email. Personal Care Support experience is an asset. Compensation The target hiring range for this position is $24.47 to $30.59 with a maximum of $36.71. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
