Care Coordinator (Full-Time, Temporaryfrom immediately, to February 5, 2027; Location: Haileybury) , you willassess and determine patient care needs and eligibility, provide access andreferrals to community services, and engage with patients, caregivers and otherhealth care practitioners. Whether you work in our office, in thecommunity, or a health care facility – you will play a lead role in providing connected,accessible, patient-centered care – and be supported by our collaborative teamthat includes over 9,000 regulated health care and other professionals. A s a valued team member, your mission will be tohelp our patients be healthier at home, while you benefit from our supports for professionalgrowth, personal wellness and work-life balance. What will you do? In collaboration with patients and their families, assess careneeds, determine eligibility for services,



and develop individual care andservice plans Linkpatients with service providers Coordinate and monitor care plan delivery Establish a helping relationship with patients and their families Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected What must youhave? Membership, in good standing, with the applicable regulatory body: College ofNurses of Ontario Collegeof Physiotherapists of Ontario Collegeof Occupational Therapists of Ontario OntarioCollege of Social Workers and Social Service Workers Recentexperience in community health or a related field Knowledgeof the health care delivery system and community resources Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment Establishedability to accurately complete required documentation, reports and forms Avalid driver’s license and access to a reliable vehicle Advancedoral and written proficiency in English and French is essential Bilingual candidates with advanced oral and written proficiencyin English and French will be given first consideration; however, lessqualified candidates who don’t meet the language requirements may beconsidered. We have amandatory COVID-19 vaccination policy.



As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination statusprior to start date. What would give you the edge? Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics Casemanagement experience or recent related community experience Abilityto speak French or another second language French Proficiency:
