This full time position involves assessing and coordinating health services for clients aged newborn to 25 years. Key Responsibilities:
• Assess, monitor, and develop care plans for children
• Coordinate home health services with families and clients




• Ensure quality patient care and continuous improvement
• Communicate effectively regarding client conditions and care plans
• Participate in on-call rotation and team meetings
Requirements:
• Registered with the College of Nurses of Ontario
• Valid First Aid or CPR certification
• Nursing diploma from a recognized institution
• Experience in community or acute care is an asset
• Valid Ontario driver's license and reliable vehicle
