Maidstone, Canada Candidates who applied for this post are required to liaise with community care providers to obtain mental health histories and facilitate transitional care at discharge working at this post. They’re appointed at this post to lead case management by initiating patient-specific plans of care working at this post. People who apply for this post are also required to coordinate care with other members of the healthcare team, such as primary care, psychiatry, social work and psychology working at this post.
Candidates who applied for this post are also required to utilize the sound knowledge of crisis prevention and intervention working at this post. Candidates must be registered as Registered Nurses with the College of Nurses of Ontario and be in positive standing to be eligible for this post. They must be registered for a mental health nurse certification program upon job offer and complete certification requirements within a specified period of time to get preferred for this post. They must be able to work collaboratively with others, such as the inter-professional team, correctional staff and community agencies. Candidates applying for this post must have effective patient advocates and can advocate for and encourage patient participation in their plan of care. They must have oral and written communication skills to clearly communicate care and progress through documentation to be preferred for this post.
