As a Clinical Guide Population Health RN, you will assess member health needs and connect them with high-quality care providers. Working closely with the Sr. Clinical Manager, your focus will include care planning, education, and reducing unnecessary healthcare costs. Engage with members to identify barriers while promoting healthier lifestyle choices.

Key Responsibilities:



• Assess severity of health concerns through clinical triage • Leverage data analytics for member outreach efforts • Identify and address care access barriers for members • Manage clinical documentation and member responses • Develop robust collaborative relationships within the team

Requirements: • RN or BSN degree with active license in compact state • 5+ years experience in direct patient care • Previous experience in population health management • Desired triage nursing skills • Experience in care management and chronic conditions
