Key Responsibilities:
• Conduct detailed client assessments
• Collaborate on primary care and medication processes




• Offer wound care and chronic disease management
• Implement rehabilitative services in the home
• Execute palliative care programs effectively

Requirements:
• Must hold a current Registered Practical Nurse license
• Preferably experienced in community health
• Valid driver's license and dependable vehicle necessary
• Satisfactory Criminal Records Check mandatory
• Capable of wearing Personal Protective Equipment (PPE)
