Lead rehabilitation efforts as a Rehabilitation Services Coordinator with March of Dimes Canada in Simcoe County. This full time role is essential in assisting clients with disabilities to achieve independence.

In this 12-month contract, you will develop and implement Individual Rehabilitation Plans tailored to client needs. Engaging with community resources and supervising rehabilitation workers will be fundamental aspects of your responsibilities. Regular assessments and progress reporting will help ensure that services meet established targets.

Key Responsibilities:
• Create and execute tailored rehabilitation strategies
• Track client progress through assessments and reporting
• Collaborate with Behaviour Consultants and associated staff




• Facilitate service delivery in various community settings
• Conduct follow-up assessments to evaluate development

Requirements:
• Preferred post-secondary education in Human Services
• Minimum three years working in a relevant field
• Strong skills in Microsoft Office applications
• Proven leadership in supervising teams
• Excellent communication and organizational abilities
