As a Nephrology-focused Registered Practical Nurse in the D2 unit, you'll deliver person-centred care in an acute setting. Collaborate with healthcare professionals to address the complex needs of patients.
This opportunity involves working independently while partnering with Registered Nurses to provide comprehensive care to patients with diverse health challenges. Engage in assessments, create care plans, and ensure quality improvements while committing to ethical nursing practices.
Key Responsibilities:
• Identify changing needs of patients and families
• Conduct assessments and develop care plans
• Implement interventions and measure patient outcomes
• Maintain a focus on quality improvement in practices
• Collaborate with the interprofessional healthcare team
Requirements:
• Registration in good standing with the College of Nurses of Ontario
• Current CPR certification from the Heart and Stroke Foundation
• RPN diploma preferred with 6-12 months of acute care experience
• Completion of the Sunnybrook Advanced Assessment Course
• Strong critical-thinking and organizational skills
