With an emphasis on individualized care plans and a supportive team environment, you'll develop a comprehensive skill set and observe remarkable patient transformations.About this team:Our specialized 16 bed inpatient reactivation program is located on the beautiful and quaint UHN-RCC campus. The Sinai Health RCC program is designed for individuals who present with decreased mobility, independence and/or function as a result of an acute illness or surgery. We focus on patients with potential for functional improvement, providing tailored care plans and utilizing state-of-the-art tools to support their recovery journey. Depending on the reactivation goals patients typically experience an average length of stay of 30 to 60 days. You will collaborate with a high performing multi-disciplinary team including Registered Nurses, Registered Practical Nurses, Nurse Practitioner, OT, PT, SW,
and SLP to provide high quality and safe patient care.In this role you will:Possess the requisite knowledge, skills, judgment and attitude to deliver competent PTA/OTA careAssist Physiotherapists and Occupational Therapists with direct patient care including but not exclusive to: lifts, transfers, ambulation, basic and instrumental activities of daily living.Supervise patients in completion of routine exercise programs in both a group and individual setting as defined by the PT or OT.Documentation in electronic health recordMaintenance of therapeutic equipmentDemonstrate accountability and responsibility for your decisions and actions and practices within your own level of competenceParticipate in Interprofessional rounds and discussionsWork collaboratively with patients, families, caregivers and the interdisciplinary healthcare teamDevelop expertise in your area of practice through self-reflection, self-learning, specialty certification and implementation of best practice guidelines.Be committed to patient safety and quality responsibilities in accordance with hospital vision,
policies and proceduresContribute to achieving Sinai Health’s strategic and operational priorities through a commitment to continuous quality improvement, patient/staff safety, professional development and continuing educationContribute to Sinai’s work by participating actively in hospital committees and other groups to represent the PTA/OTA perspectiveAssist with other department/unit responsibilities or duties, as requiredMentor colleagues and new hires as appropriateParticipate in teaching of students from affiliated institutions.Job Requirements:Mandatory:Graduate of an accredited community college occupational therapy/physiotherapy assistant programA minimum of one (1) year of experience working in RehabPreferred:Experience using an electronic health recordSkills and Knowledge:Demonstrates good understanding of the principles of rehabilitationAbility to cope with the physical demands of the positionAbility to work independently and with a teamFamiliarity with common Rehabilitation equipment, exercises and treatments.Strong disposition planning skills and knowledge of community supportsDemonstrated ability in providing quality patient-focused care and addressing the fundamental care needs of patients and their familiesDemonstrated leadership skillsExcellent problem solving and critical thinking skillsExcellent oral, written and verbal communication and interpersonal skillsProficiency in EnglishDemonstrated satisfactory job performance and attendance historyComputer proficiency and/or Basic to Intermediate Microsoft Office skills (e.G.
