This full-time position involves providing comprehensive social work services tailored to the needs of patients and their families facing chronic illnesses. Key responsibilities include psychosocial assessment, risk screening, counselling, and effective resource navigation. Partnering with healthcare teams, you'll ensure supportive care, advocating for patient needs while supporting their journey through the complexities of managing kidney disease.
Key Responsibilities:
• Perform comprehensive psychosocial assessments and support
• Offer individual and family treatment solutions
• Engage in risk screening and case management tasks
• Coordinate discharge planning and transitions contextually
• Maintain awareness of community resources and public initiatives
Requirements:
• Bachelor’s degree in Social Work is essential
• Active or eligible ACSW registration required
• 1-2 years RSW experience in renal care or chronic illness
• Demonstrated cultural awareness and teamwork skills
• Current Basic Life Support (BLS-HCP) certification
