Become part of AVI Health & Community Services as a Licensed Practical Nurse dedicated to providing healthcare in Victoria. Focus on participant-centered care for individuals experiencing complex health issues.

In this SAFER Nurse position, you'll utilize your expertise to assess and educate participants with poly-substance use. Collaborating with an interdisciplinary team, you'll develop care plans and provide harm reduction support in various community settings, empowering individuals on their health journey.

Key Responsibilities:
• Deliver healthcare support for acute and chronic conditions
• Work independently and as part of the SAFER team
• Offer resources and referrals for mental and sexual health




• Collaborate with community partners for integrated care
• Engage in staff training and community networking

Requirements:
• Licensed Practical Nurse with 1-5 years of relevant experience
• Background in harm reduction and substance use services
• Solid understanding of public health equity models
• Experience working with marginalized communities
• Solid advocacy, communication, and crisis skills

Contribute to meaningful health improvements for vulnerable populations in your role at AVI.
