Promote independence for adults with disabilities with PACE as a Community Resource Navigator. This full time position focuses on coordinated service delivery and client-centered support, ensuring community engagement.
In your role as a Community Resource Navigator at PACE, you will directly collaborate with clients, caregivers, and diverse teams. Your main goal will be to assist individuals in successfully navigating health and social service systems. Emphasizing personalized planning and community partnerships, you’ll advocate for increased resources and support, significantly impacting clients' lives.
Key Responsibilities:
• Lead individualized support planning sessions
• Navigate and coordinate service transitions
• Cultivate relationships with community partners
• Provide information for client resource accessibility
• Support program initiation and evaluation activities
Requirements:
• Education in Social Work or a related field
• 3-5 years in a relevant community service role
• Background in client advocacy and system navigation
• Familiarity with accreditation and service standards
• Excellent communication and organizational skills
