Take on a fulfilling role as a Case Manager with the Canadian Mental Health Association in Haldimand Norfolk. This full time position focuses on providing community-based interventions for individuals facing mental health challenges.
The role entails supporting a caseload of over 20 clients primarily in Brant County, using a Psychosocial Rehabilitation and Recovery model. A minimum of a BA or BSW is required along with relevant experience in community mental health services. You will need reliable transportation for client visits and occasionally work evenings or weekends, depending on client needs.
Key Responsibilities:
• Manage a caseload of 20+ community clients
• Conduct assessments and progress reports for clients
• Support mental health and concurrent disorders treatment
• Implement community-focused rehabilitation interventions
• Maintain effective communication with clients and stakeholders
Requirements:
• BA or BSW with relevant experience
• Knowledge of local mental health services
• Current emergency first aid/CPR certification
• Proficient in Microsoft Office and client data systems
• Clean criminal records check required
