Key Responsibilities:
• Conduct scheduled visits in community settings
• Provide ethical and safe nursing care to patients
• Use assessment techniques to gauge needs




• Adjust care plans based on continuous monitoring

Requirements:
• Current CNO registration as a Registered Practical Nurse
• Driver’s license and vehicle access are required
• Experience in community care is advantageous
• Must be flexible and reliable
• Experience working with vulnerable populations preferred
