About the Transitional Care Unit (TCU)
The Transitional Care Unit (TCU), located at the Alternative Healthcare Facility in Amherstview, is a 120-bed program supporting patients who require ongoing care while awaiting their next stage of care. Patients may be:
Working toward increased independence through rehabilitation
Receiving palliative and comfort-focused care
Our Nurse Practitioner-led model promotes a collaborative approach that allows team members to build meaningful relationships with patients and contribute to their ongoing care journey.

What You'll Do
As a Registered Practical Nurse, you will:
Assess, plan, implement, and evaluate patient care within the RPN scope of practice
Deliver evidence-informed nursing care focused on patient goals and outcomes
Monitor patient progress, identify changes in condition, and collaborate with the healthcare team as needed
Develop and coordinate individualized care plans
Support patients and families through rehabilitation, transitional care, and palliative care journeys
Delegate, teach, and supervise unregulated care providers
Participate in discharge planning and care coordination
Collaborate with physicians, allied health professionals, patients, and families to ensure seamless patient care
Contribute to quality improvement initiatives that enhance patient experiences and outcomes
Mentor students and team members while fostering a positive practice environment

Required Qualifications
Diploma from a Registered Practical Nurse program
Current registration in good standing with the College of Nurses of Ontario (or eligible for registration)
Demonstrated knowledge of CNO standards and RPN scope of practice
Excellent assessment, communication, and organizational skills
Strong problem-solving and decision-making abilities
Basic computer proficiency
Ability to work effectively in a fast-paced healthcare environment
Proven ability to attend work regularly
Satisfactory Criminal Record Check and Vulnerable Sector Screening
