Become an Integrated Primary Care Coordinator with Carefirst FHT to enhance patient access and engagement with primary care resources. Collaborate with a dedicated team in a community-driven setting. This role focuses on identifying unattached patients in the EYRND communities and connecting them to ongoing primary care. You will work to facilitate improved communication between care organizations, enhancing the overall health care experience for community members.

Your role will be pivotal in executing strategies for patient attachment and engagement. Key Responsibilities:

- Identify unattached patients and coordinate care connections
- Engage with community service providers for outreach
- Host community activities to raise awareness




- Collaborate with various health care teams for effective referrals
- Support primary care integration projects and evaluations Requirements:
- 5 years relevant experience in health care
- Undergraduate degree in a health-related field
- Solid organizational and community engagement skills
- Familiarity with Ontario health strategies
- Excellent problem-solving and communication capabilities Drive impactful health care initiatives while connecting patients to essential services in your community.
