Join St. Joseph’s Home Care as a Community Dietitian and deliver tailored nutrition care across HNHB regions. Support diverse client dietary needs while collaborating with healthcare teams. In this part time role, you will work with the Integrated Comprehensive Care Program (ICCP) to assess and address various nutritional challenges faced by clients.

By conducting thorough assessments and providing education, you’ll guide clients and support staff involved in their care initiatives. Your efforts will promote health and ensure clients navigate their dietary requirements effectively. Key Responsibilities:

- Perform comprehensive nutritional assessments
- Monitor and update client dietary requirements
- Participate in interdisciplinary team reviews




- Teach clients about dietary needs and restrictions
- Advocate for clients’ nutrition-related community resources Requirements:
- Relevant university degree in Nutrition required
- Completion of dietitian internship mandatory
- Registered with the College of Dietitians required
- Three years’ experience in a similar setting preferred
- Experience with geriatric and mental health populations an asset Contribute to meaningful dietary interventions and community health improvements at St. Joseph’s Home Care.
