Your role emphasizes relationship building and utilizes a client-centered approach, addressing social determinants of health and promoting health equity. Key Responsibilities:
- Deliver support and recovery services based on individualized service plans
- Engage with recent and existing clients to facilitate skill-building
- Advocate for clients using a non-judgmental and culturally appropriate manner
- Collaborate within an interdisciplinary team to achieve service goals
- Apply harm reduction and trauma-informed practices for client care
Requirements:
- Valid Driver’s License and access to reliable transportation
- Minimum 2 years’ experience with community programs
- Proven skills in crisis management and problem-solving
- Understanding of Indigenous cultures and their impacts
- Strong communication and computer proficiency
