Start Date: As soon as possible Contract: Permanent Assess, monitor, and document residents' nutritional and hydration status. Develop, evaluate, and monitor individualized nutrition care plans for residents. Provide counselling and education to residents, and monitor high risk residents. Participate in interdisciplinary care conferences and continuous quality improvement initiatives.
Registered
Dietitian eligible for membership with the College of Dietitians of Ontario.
Experience in a long-term care setting is an asset. Must have at least two doses of the COVID-19 vaccine. Must have a TB test and Vulnerable Sector Check issued within the past six months.
