The Carefirst Family Health Team (FHT) is seeking full-time permanent Director of Clinical Operations/Clinical Lead (Nursing) to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.

About Us

Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, service navigators, dieticians, physiotherapists, and medical receptionists. We have a robust Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.

Our “Hub & Spoke Model” and strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with Carefirst Seniors & Community Services Association to provide truly integrated services.

On‑site & Integrated Programs/Services include:

- Ontario Health atHome (formerly Home and Community Care Support Services)
- MINT Memory Clinic
- Ontario Structured Psychotherapy (OSP) Program
- Specialist Clinics: psychiatry, cardiology, endocrinology, optometry, geriatric medicine, gynecology, chiropody, rheumatology, physiotherapy, and audiology

Clinic Locations

We currently operate five well-established clinics across Scarborough, Markham, Richmond Hill, and Stouffville and continue to expand in Markham to support continued growth and community needs.

Current clinic locations:

- 300 Silver Star Blvd, Scarborough (Midland / McNicoll)
- 420 Hwy 7 East, Richmond Hill (West Beaver Creek / Hwy 7)
- 360 Highway 7 East, Richmond Hill (Highway 7 / Chalmers Rd)
- 105 Gibson Centre, Markham (Steeles Ave East/Warden Ave)
- 37 Sandiford Drive, Stouffville (Stouffville Main St/Sandiford Dr)

Upcoming new clinic locations:

- Unionville Commons Seniors Hub, Markham (Unionville Main Street/Highway 7 East)

Employees will be assigned to a location that aligns with the operation needs, with occasional rotation to support clinic operations.

We are currently seeking a Director of Clinical Operations/Clinical Lead (Nursing) to fill a new position at our Family Health Team. As Carefirst continues to grow, we are creating a new senior leadership role dedicated to nursing excellence and clinical operations.



Reporting to the Executive Director and working in close partnership with the Administration Lead, the Director of Clinical Operations (known day-to-day as the Clinical Lead) will lead nursing team across all sites, building the clinical systems and standards that support safe, high-quality, patient-centred care at scale.

This is a hands-on leadership opportunity for a clinically credentialed, systems-minded leader who wants a genuine seat at the table in a value-driven, expanding organization.

Terms: Full Time Permanent

Pay range: $98,000 - $118,000 annually

Responsibilities:

Senior Leadership & Organizational Partnership

- Share collective accountability with the Executive Director and Administration Lead for Carefirst's strategic, operational, financial, quality, and people outcomes.
- Serve as a full member of the senior leadership team, contributing to organizational strategy, planning, budgeting, and decision-making.
- Work closely with the Administration Lead to balance organizational priorities, resources, and workload and foster a unified leadership culture.
- Provide reciprocal leadership coverage during planned and unplanned absences to ensure continuity of decision-making and operations.
- Support organizational growth, change management, new hub openings, and program implementation.

Nursing Leadership & Practice Excellence

- Provide direct leadership and clinical oversight to RNs and RPNs across all hub sites and, through them, support consistent nursing practice across the organization.
- Lead, coach, mentor, and develop nursing leaders and staff, fostering accountability, collaboration, continuous learning, and clinical excellence.
- Oversee nursing performance management, including coaching, performance improvement plans, and progressive discipline where warranted.
- Serve as the clinical escalation point for complex nursing practice, scope-of-practice, and clinical judgement issues.




- Ensure consistent clinical protocols, triage standards, and EMR documentation practices across sites.
- Ensure nursing practice complies with applicable qualified standards, legislation, and organizational policies.

Clinical Systems, Quality & Continuous Improvement

- Design, implement, and continuously improve scalable clinical systems, workflows, standard operating procedures, policies, and protocols.
- Establish and monitor clinical KPIs and use data to drive improvements in areas such as patient flow, triage timeliness, program delivery, immunization coverage, and incident trends.
- Lead nursing-related root-cause analyses, investigations, and quality improvement initiatives in collaboration with internal quality and decision-support resources.
- Promote consistent clinical standards and high-quality patient care across a growing multi-site organization.
- Provide regular reporting and insights to the Executive Director on clinical performance, quality, risks, and emerging priorities.

Requirements:

- Registered and in good standing with the College of Nurses of Ontario (RN)
- BScN required; Master's degree in Nursing, Health Administration, or a related field an asset
- 5-7+years of progressive nursing leadership experience, including direct people management and formal performance management
- Experience in a primary care or community health setting strongly preferred
- Excellent communication, negotiation and interpersonal skills, with a collaborative, team-oriented approach
- Fluent in English and a second language that reflects the community served, such as Cantonese, Mandarin, Tamil and Korean, is an asset
- A valid vulnerable sector police record check completed within one year
