As a Nurse Practitioner or Physician Assistant, you will establish long-term relationships with patients and support their health needs with evidence-based care. Key Responsibilities:
• Deliver in-home patient assessments and wellness visits • Provide education on health management and preventive care • Engage in virtual team discussions for integrated care • Coordinate with specialists and document care effectively • Perform point-of-care testing and prescribe medications
Requirements: • Active NP or PA license and board certification • Two years of direct patient care experience • Current DEA certification and BLS certification • Familiarity with EHR systems and HIPAA regulations • Initiative and independence in decision-making
