Key Responsibilities:
• Conduct in-home assessments and create personalized care plans • Administer medications and deliver clinical interventions • Monitor patient progress and adjust care plans accordingly • Collaborate with interdisciplinary teams for seamless care • Educate clients and caregivers on health management
Requirements: • Graduate of an accredited Nursing program (RN) • Current registration with CNO • 2-5 years of recent nursing experience • Valid G driver’s license and reliable vehicle • Willingness to travel in various weather conditions
