The Lakeland Centre for Fetal Alcohol Spectrum Disorder seeks an Outreach Navigator for its St. Paul office. This full-time role involves case management and community engagement for families impacted by FASD.

In this position, the FASD Client Resource Navigator will liaise with families, schools, and health services. You will manage various cases, ensuring comprehensive support and advocacy for individuals. This role demands adaptability and frequent travel within the community to meet the needs of clients.

Key Responsibilities:

- Assist families with post-diagnostic follow-up
- Facilitate connections to support services
- Provide education to caregivers and community agencies
- Follow organizational policies and maintain client confidentiality
- Engage in community outreach and presentations Requirements:
- Degree in human services or related field
- Three years of field experience with communities
- Robust written and verbal communication skills
- Valid driver’s license and insurance
- Competence in Microsoft Office, particularly Outlook and Word Utilize your skills to make a lasting difference for individuals affected by FASD at the Lakeland Centre.
