Become an RN Care Coordinator focused on quality care for children at the Child Health Pavilion. In this position with PFK Care Coordination, you will collaborate with multidisciplinary teams to develop individualized care strategies for children with special needs.
Your role will emphasize patient safety, advocacy, and quality care across various settings, including outpatient and home visits. This is a critical position for ensuring continuity during patient transitions. Key Responsibilities:
- Develop and implement personalized care plans
- Facilitate transitions of care for continuity
- Collaborate with families and healthcare professionals
- Promote advocacy based on ethical nursing standards
- Coordinate post-discharge care for patients Requirements:
- Bachelor's degree in Nursing required
- RN license from the Ohio Board of Nursing
- At least 3 years of nursing experience required
- Obtain BLS certification within first 60 days
